Résumé
The story of therapeutic patient education (TPE) has been strongly influenced by evolution of theories and concepts in the fields of medicine, psychology, and pedagogy. TPE has always existed in the form of advices, but actually it was born in 1922 with the first treatment with insulin. It has had three periods. The first period spans half a century; pedagogy was vertical, authoritarian, passive. The second period began in 1970s; it was characterized by the development of a humanist, active, constructivist pedagogy. The third period, since the end of the 20th century, is dominated by the skill-based approach and by the management by objectives. This operative approach underestimates the importance of emotional experience of patients. Will TPE became a mere supplementary procedure, or will it define another medicine?
L’histoire de l’éducation thérapeutique du patient (ETP) a été fortement influencée par l’évolution des théories et des concepts en médecine, en psychologie, et en pédagogie. L’ETP a toujours existé sous forme de conseils, mais elle est réellement née en 1922 avec les premiers traitements par l’insuline de patients diabétiques de type 1. Elle a connu trois périodes. La première période s’étend sur un demi-siècle, la pédagogie était verticale, autoritaire, passive. La deuxième période débute dans les années 1970 ; elle est marquée par le développement créatif d’une pédagogie humaniste, active, constructiviste. La dernière période, depuis la fin du 20e siècle et ce début de 21e siècle, est dominée par l’approche par compétences et par le management par objectifs. Cette approche opératoire sous-estime l’importance du vécu émotionnel des patients. L’ETP deviendra-t-elle une simple prestation supplémentaire, ou définira-t-elle une autre médecine ?
